Upon receipt of your order we may need any or all of the following;
Federal Tax ID Number, States Sales Tax Certification, Business License as well as any other proof of identity along with copies of these documents. If needed, Related DEA or FDA Licences or Certifications may have to be provided. Lab Express may also request any other documents that may be deemed as necessary.
